Why It Matters:
Chronic conditions such as diabetes, heart disease, hypertension, and obesity continue to affect families across McHenry County. Many individuals face barriers to care, including limited access to preventative services, lack of insurance, language barriers, or uncertainty about where to turn for help.
We exist to bridge those gaps.
Through no-cost health screenings, education, and follow-up, we help individuals understand their health status and connect to the care and resources they need to move forward with confidence.
