Cultivating Health Ministries provides comprehensive, no-cost health screenings with education and follow-up support throughout McHenry County. We do not diagnose or treat individuals.

Our collaborative, community-based model ensures that each participant receives more than a test result. We provide personal guidance, referrals, and connections to ongoing healthcare.

Immediate Results and Education

Test results are available at the screening event. The collaborative team providing the screenings includes nurses, doctors, medical assistants, behavioral health clinicians, nutrition educators, and other community partners.

During your screening, results are explained clearly and compassionately by licensed physicians, nurse practitioners, and diabetes educators. Participants receive practical education about what their results mean and what steps may be recommended, but no diagnosis or treatment is given. Instead, referrals are made to area healthcare providers.

When appropriate, participants may receive a blood pressure monitor and/or a glucometer.

This immediate support allows individuals to begin monitoring their health right away.


Evidence-Based Referrals and Community Connections

Screenings are the first step.

Based on results, participants may be referred to:

  • Evidence-based nutrition and diabetes education classes

  • Primary care providers

  • Behavioral health services

  • Community health resources

  • Food and nutrition assistance programs

Individuals receiving medical devices after screening receive training from our community health volunteers on how to properly use their device.

We strive to promote health literacy and eliminate inequities in McHenry County by achieving transformative healthcare outcomes through innovative, evidence-based health screenings and education, premier partnerships with community organizations, and seamless access to healthcare providers.

Follow Up Care Matters

Follow-up is an essential part of our model. Follow-up calls are provided after the screening event, providing additional educational and referral support.

These calls allow our team to:

  • Answer additional questions

  • Review recommendations

  • Ensure needs are being met

  • Provide encouragement and clarification

  • Provide support to improve their health

For individuals with positive screening results, a 3 to 6 month follow-up is strongly recommended to monitor progress and provide continued guidance.

We believe health improvement is a process, not a single event.
